Letters of Introduction

Students please return the forms attached to your letters of introduction to your parents and yourself. These forms are to be filled out completely and handed in to me by Monday, September 14, 2009. If you have lost the parent letter of introduction or student letter of introduction, please click on the appropriate link highlighted above and you will be provided an electronic copy of the document. Students you are to then access the copy, fill out the form, print it out and then hand it in completed to me.
